Banker-Johnson, 508-274-4360, seastars_preschool@msn.com By signing the end of this indoor IPM plan, the IPM coordinator, John Johnson, of this Daycare and the Pest Management Professionals described above acknowledge, and agree to the terms of this INDOOR integrated pest management plan. A .INTRODUCTION In compliance with the Act Protecting Children and Families from Harmful Pesticides, Sea Stars Preschool on 8/22/2009 1:40:00 PM has prepared the following indoor IPM plan. By centralizing all of the information about this facility’s pest management practices the plan serves as a guide to direct this facility’s IPM coordinator, John Johnson , about pest control and pesticide use. This plan describes the pest management practices for indoor areas of the Sea Stars Preschool and clearly states it’s pesticide use policies. A copy of the plan has been filed with the Massachusetts Department of Agricultural Resources (MDAR), and at least one printed copy must be kept on site and made available to the public upon request. Objectives The objectives of the integrated pest management program conducted at the Sea Stars Preschool are listed below. -

| Manage pests that may occur on facilities to prevent interference with the learning environment of the students. | -
| Provide the safest playing or athletic surfaces possible. | In light of these objectives, the Sea Stars Preschool has selected the following as it’s IPM policy statement: B.POLICY STATEMENT It is the policy of this school to implement Integrated Pest Management procedures to control structural and landscape pests and minimize exposure of children, faculty, and staff to pesticides. C.IPM COMMITTEE The tasks set before an IPM committee are to: -
| Develop an IPM plan. The IPM plan is in essence, a document that describes the organization and implementation of IPM on school grounds. | -
| Evaluate progress of the IPM program. | -
| Communicate about IPM - Facilitate communication within the school about IPM practices. | -
| Assist in development of contract specifications. | -

COMMUNICATING IPM WITHIN THE FACILITY The Pest Management Professional communicates with the IPM coordinator of the facility. The IPM coordinator then posts this information in a common viewing area dedicated to the subject of integrated pest management. In addition, the IPM coordinator also communicates information to the staff supervisors who then distribute the information to all the staff and occupants concerned. Staff/Students communicate in writing with an administrator who then passes the information onto the IPM coordinator when necessary. E. EDUCATION AND TRAINING OF FACILITY OCCUPANTS & STAFF - IPM Coordinator, School Administrator, and all school staff.
- Training content will cover pest-reporting procedures and pest prevention strategies including the following: Proper sanitation, Pest sighting/inspection sheets, application records, blank forms and waivers, chemcial fact sheets, copies of notifications, compliance checklist, labels and materials safety data sheets, copies of "Children''s and Families Protection Act."
- Yearly training seminar on IPM for all Sea Stars program staff, and annual reminders sent to staff via email concerning IPM training.
- The IPM Coordinator will present the training each September/the beginning of the School Year. The method used to evaluate the training will be a quiz and a mock reporting exercise.
- School will have the IPM Plan posted in a conspicuous location on the school''s Family Information Center. Along with the IPM will be posted suggestions for keeping school areas from pests.
F. INDOOR MONITORING The IPM committee will evaluate the plan monthly. When pests are present, Sea Stars Preschool has chosen an INDOOR monitoring schedule that consists of daily inspections. When pests are absent the INDOOR monitoring schedule will consist of monthly inspections. The following technique will be used to monitor for pests: Reports from facility personnel (pest reporting forms) would prompt the IPM coordinator to contact the facility’s Pest Management Professional who would then conduct a facility audit. G. COURSE OF ACTION TAKEN FOR INDOOR PESTS The School's IPM approach to managing the indoor pests includes the following actions: SCHOOL PEST DESCRIPTION | No pests identified. | SCHOOL PEST LOCATION DETAILS | No pests identified. | SCHOOL PEST ACTIVITY | No pests identified. | MONITORING/INSPECTION | Monthly monitoring and recording of any pests noted in Pest Management Inspection Log. | ELIMINATING ACCESS | All window screens are checked regularly for holes/damage and repaired if necessary. All entry points to the program area are checked for signs of pest presence and sealed immediately if necessary. | ELIMINATING SOURCES OF FOOD AND WATER | All food is kept in air-tight, sealed, and secure containers in shelving off the floor, or in a well-insulated/sealed refrigerator. All non-water beverages will be stored in the same refrigerator. All drinking water sources will be directly from the tap or some other sealed water bottle source. | ELIMINATION OF SHELTER AND HARBORAGE | We will avoid storing food or potential nesting materials in any cardboard boxes for any extended period of time. Rodents can easily chew through cardboard and insects find good harborage in the folds of boxes and beneath them. If storage of cardboard boxes cannot be avoided, we will store boxes in such a way to allow for inspection and to avoid creating pest harborage. If possible we will use shelving units that are kept away from walls. This allows for inspection and to avoid creating harborage areas. Non-food items such as linens, glassware, and dishes will be stored as far away from food items as possible. No edible foodstuffs are left uncovered or exposed overnight. Kitchen items and surfaces are not to be left unwashed overnight. Soiled clothes will be kept in a sealed container and washed frequently. | NON-CHEMICAL CONTROLS | 1. Sanitation - Enforce quality sanitary practices daily, such as emptying all trash receptacles, washing all utensils and dishes, and wiping down all surfaces that food has come in contact with, etc.. 2. Physical exclusion of pests. 3. Mechanical controls - Traps, etc.. | PESTICIDE USE ATTESTATION: | Pesticides are only applied by a certified and/or licensed applicator. | Pesticides are used only when monitoring has shown that pests are present and when the use of the pesticide is justified or in the case of an emergency situation. | Only pesticides allowed under the Children's and Families' Protection Act are used indoors. | H. RECORD KEEPING In the case of Sea Stars Preschool, INDOOOR monitoring records will be maintained through the following technique: The use of forms which will be filled out by the person monitoring the facility I. Banker-Johnson has assigned committee member John Johnson with the responsibility of applying for an emergency waiver. K.
