Applicators are required to provide the following information to schools, day care centers and school age child care programs. Applicators should download the following documents, complete the Standard Written notification form and provide the documents to the school, day care center or school age child care program. The school, day care center or school age child care program is then responsible for notifying parents, staff members and students.
